Queen's Health System Clinical Coordinator,Case Manager/UM Reviewer RN (Big Island, Full-Time, Day) in Kamuela, Hawaii
RESPONSIBILITIES
I. JOB SUMMARY/RESPONSIBILITIES:
• Provides clinical consultation as a Registered Nurse (RN) with the Patient Care Team to facilitate coordination of care across the continuum for selected complex patients.
• Partners with Patient Care Team; maintains accountability for the delivery of patient care and oversees care throughout patient’s admission and return to community setting as applicable.
• Key responsibilities include, but are not limited to, case finding and assessment, patient/family education and advocacy, resource management and discharge planning in an acute care, long-term care setting, or community setting, as assigned.
• Coordinates, implements and facilitates key clinical performance improvement efforts including, but not limited to: disease management, peer review, quality and risk management focus studies outcomes projects and research.
• Accountable for the development, organization, and implementation of utilization review (UR) processes in order to achieve efficient cost effective resource management while maintaining quality of care.
• Conducts medical record reviews to ensure safe and effective patient care, customer satisfaction and quality outcomes.
II. TYPICAL PHYSICAL DEMANDS:
• Essential: standing, sitting, walking, stooping/bending, kneeling, squatting, finger dexterity, seeing, hearing, speaking, lifting and carrying usual weight of 10 pounds, reaching above, at and below shoulder level, repetitive arm/hand motions, static gripping of an object for prolonged periods.
• Operates typewriter, computer, telephone, printer, calculator, and copier.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Work environment may be stressful due to deadlines and workload.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Bachelor’s degree in Nursing.
• Current Hawaii State licensure as a Registered Nurse.
• Current BLS/HCP certification.
• Current certification as a Certified Case Manager (CCM) through CCMC preferred.
B. EXPERIENCE:
• Two (2) years clinical nursing experience.
• Knowledge of performance improvement, databases/information systems preferred.
• Demonstrated experience with computer word-processing, spreadsheet, and presentation applications.
