Queen's Health System Patient Navigator Associate - SURG Breast POB2 803 (Full-Time, 40, Day) in Honolulu, Hawaii
RESPONSIBILITIES
I. JOB SUMMARY/RESPONSIBILITIES:
• Supports oncology patients and their caregivers through the health care system by providing access to supportive services and helping them overcome barriers to care.
• Assists and navigates patients with identified external barriers as related to transportation, lodging, travel, and interpreter arrangements.
• Performs duties and responsibilities in accordance with policies and procedures and consistent with the goals and objective of the department and The Queen’s Medical Center (QMC).
II. TYPICAL PHYSICAL DEMANDS:
• Required: finger dexterity, seeing, hearing, speaking.
• Frequent: standing, walking, sitting.
• Occasional: kneeling, climbing stairs, walking on uneven ground, reaching at and below shoulder level, stooping/bending, squatting, lifting usual weight of 10 pounds up to 25 pounds, pushing/pulling usual weight of 10 pounds up to 25 pounds, carrying usual weight of 5 pounds, reaching above shoulder level.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• May be required to work at The Queen’s Medical Center – West Oahu.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Associate’s degree in a health related field; or two (2) years work experience in healthcare may be substituted for the educational requirement.
B. EXPERIENCE:
• Prior work experience in customer service or direct patient interaction.
• Knowledge of Microsoft Office applications, i.e. Word, Excel, and PowerPoint.
